We are now offering a new service to the community. Dr Palley is a Civil Surgeon for the U.S. Citizenship and Immigration Services, and will be conducting medical examinations for individuals seeking a status change.
For those wishing to do this medical examination, you will need to
1. Bring picture ID (can be a passport, driver’s license, work authorization card, etc.)
2. Bring any medical records if they are easily available
3. Bring an interpreter if needed for any language other than Englich or Spanish. We are fluent in both English and Spanish, but if another language is spoken, you will need to bring an interpreter.
4. Bring all vaccination (immunization records)
5. Bring your $300 fee in cash. This fee includes the medical exam and all required labs, as well as completion of the I-693 form. Additional labs may be recommended which are not included in this fee. Required vaccinations are not included in this fee. All fees must be paid in full prior to the I-693 form being released.
6. Most people will need to have a blood draw for required testing. Some adults may be required to provide a urine sample. Which tests are run are based on the USCIS medical testing requirements. It’s OK to eat or drink before the exam, and being well-hydrated will make it easier to obtain blood and urine specimens.
